Using form 990, since it is an IRS form it is essentially a gov’t publication w/ no author. In that case, for the works cited page, you’d start with the name of the gov’t body, then the gov’t agency, next would be the title (underlined), and finally where it was printed, which since its a gov’t doc, it was most likely the gov’t printing office (GPO) in Washington, then the year.
The end product should look like this (using the Form 990 ex):
United States Department of the Treasury. Internal Revenue Service. _Form 990_. Washington: GPO, 2007.
The dashes around “Form 990″ mean underlined, and I assumed you are using the 2007 form. For in-text citation you could abbreviate to something like (Dept. of Treasury).
